Initial Care Management Consultation

A focused 90-minute consultation to review the current situation, identify immediate concerns and priorities, and develop clear next steps. Includes professional review after the call and a personalized Clarity & Next Steps summary.

This may be a good fit if you’re unsure what to do next, managing multiple providers or services, preparing for or recovering from a hospitalization, or need an experienced RN perspective to help organize the situation.

Ongoing care management
Starting at $125/hour

Ongoing RN support for individuals, families, and residential providers who need help keeping healthcare organized, coordinated, and moving forward.

Services may include provider communication, appointment coordination, medical follow-up, care-team communication, discharge planning, equipment and supply coordination, family updates, and identifying gaps in care.

For residential providers, support may also include assistance with coordinating resident healthcare needs, organizing follow-up, communicating with medical providers, and helping ensure recommendations are carried through.

This may be a good fit if care is complex, several providers are involved, family lives out of town, or your organization needs experienced RN support coordinating care across services.
