About Manage with Care
Manage with Care provides independent RN care navigation, care management, advocacy, healthcare coordination, caregiver education, and related support services.
Services are intended to help individuals and families better understand, organize, and navigate healthcare needs and available resources.
Not Emergency or Medical Treatment
Manage with Care does not provide emergency medical services.
If you or another person may be experiencing a medical emergency, call 911 or seek immediate medical attention.
Manage with Care does not replace your physician, nurse practitioner, specialist, home health agency, emergency department, or other treating healthcare provider. Information and guidance provided through this website or educational products should not be used as a substitute for diagnosis, treatment, or individualized medical advice from your treating provider.
Professional Services
Specific care-management, consultation, navigation, or advocacy services may require a separate service agreement, consent forms, or authorization before services begin.
The scope of services will depend upon the needs identified and services agreed upon.
Manage with Care may assist with matters such as provider communication, appointment coordination, healthcare follow-up, discharge planning, resource navigation, equipment or supply coordination, caregiver education, and family communication.
Services do not guarantee a particular medical, insurance, financial, or care outcome.
Initial Consultations
Initial Care Consultations are intended to review the current situation, identify concerns and priorities, discuss available options, and develop recommended next steps.
A consultation does not establish responsibility for ongoing care management unless ongoing services are separately agreed upon.
Discovery Calls
Complimentary Discovery Calls are brief conversations intended to determine whether Manage with Care may be an appropriate resource.
Discovery Calls are not intended for detailed care planning, clinical assessment, diagnosis, or medical advice.
Scheduling, Fees & Payment
Current fees are displayed at booking or discussed before services begin.
Payment may be required at the time of scheduling. When available, payment arrangements or installment options will be governed by the terms presented at checkout or in the applicable service agreement.
Ongoing care-management fees may vary depending on the level and frequency of support needed.
Cancellation & Rescheduling
Please provide at least 24 hours' notice when cancelling or rescheduling a scheduled consultation or appointment.
Late cancellations and missed appointments may be subject to a fee as disclosed during booking.
We can adjust this section once you decide exactly what cancellation policy you want.
Client Responsibilities
Clients are responsible for providing information that is accurate and reasonably complete.
Clients remain responsible for making their own healthcare decisions and for following the recommendations and instructions of their treating healthcare providers.
Manage with Care should be informed promptly of significant changes that may affect requested services.
Communication & Secure Client Portal
Clients receiving ongoing care-management services may have access to a secure client platform for communication, documents, care information, tasks, or other resources.
Availability of portal features may vary according to the services being provided.
General website forms, standard email, and ordinary text messaging should not be used to send highly sensitive medical information unless specifically directed.
Provider & Community Referrals
Manage with Care may identify or recommend healthcare providers, home-care agencies, equipment companies, transportation services, community organizations, or other resources.
Unless specifically stated otherwise, these organizations are independent third parties and are not employees or agents of Manage with Care.
A referral or resource recommendation is not a guarantee or endorsement of the quality, availability, insurance participation, or outcome of services provided by another organization.
Insurance & Benefits
Manage with Care may assist clients with understanding or navigating healthcare and insurance processes but does not determine insurance coverage or guarantee payment of any claim, service, equipment, medication, or benefit.
Coverage decisions should be confirmed directly with the applicable insurance plan or benefit administrator.
Educational Programs & Digital Products
Courses, guides, worksheets, journals, and other educational materials are provided for informational and educational purposes.
Digital materials are for the purchaser's personal use and may not be reproduced, distributed, shared, resold, or published without written permission.
Unless otherwise stated at purchase, digital educational products are non-refundable after access has been provided.
Privacy
Your use of this website and services is also subject to the Manage with Care Privacy Policy.
Additional privacy and confidentiality provisions may apply to clients receiving professional services through a secure care-management platform.
